Patient Guideline

For a safe, comfortable, and informed hospital experience.

Admission Process

Registration

Proceed to the Admitting Section with a valid ID and referral letter or doctor's order. Fill out the admission form completely.

Medical Assessment

A nurse will assess your vital signs and confirm your attending physician's orders before room assignment.

Room Assignment

Choose your preferred room type — Private, Semi-Private, or Ward. Room availability is subject to current occupancy.

Orientation

Our nursing staff will orient you and your companion on hospital policies, facilities, nurse call system, and care plan.

Patient Rights & Responsibilities

Your Rights

  • Receive respectful and compassionate care at all times
  • Be informed about your diagnosis, treatment, and prognosis in terms you can understand
  • Participate in decisions about your own healthcare and treatment plan
  • Refuse treatment to the extent permitted by law
  • Privacy and confidentiality of your medical records
  • Access your medical records upon written request
  • Be informed of hospital charges and billing
  • Voice complaints or grievances without fear of retaliation

Your Responsibilities

  • Provide accurate and complete medical history to your healthcare team
  • Follow the treatment plan prescribed by your attending physician
  • Treat all hospital staff, visitors, and other patients with respect
  • Keep noise to a minimum and respect quiet hours (10:00 PM – 5:00 AM)
  • Settle hospital bills and financial obligations promptly
  • Inform staff of any changes in your condition immediately
  • Follow all hospital rules, policies, and safety guidelines
  • Refrain from smoking anywhere within the hospital premises

General Hospital Guidelines

Valuables & Security

ICMC is not liable for lost or stolen valuables. Please deposit cash and jewelry at the Nursing Station for safekeeping.

Mobile Phones

Phones must be set to silent mode in patient areas and the ICU. Avoid calls in areas where medical equipment is in use.

No Self-Medication

Do not take any medicines not prescribed by your attending physician. All medications are managed by our nursing staff.

No Smoking Policy

ICMC is a strictly smoke-free facility. Smoking is prohibited in all areas including hallways, restrooms, and parking areas.

Food & Diet

Outside food may be restricted depending on your doctor's orders. Follow your prescribed diet plan strictly during your stay.

Hospital Premises

Patients and visitors must wear their ID bands and visitor passes at all times. Unauthorized access to restricted areas is prohibited.

Delivery Room / OB As permitted by OB-Gyne 1 support person Subject to attending physician's discretion

Discharge Process

Doctor's Order

Your attending physician issues the discharge order. Receive instructions on medications, follow-up visits, and home care.

Billing Statement

Proceed to the Billing Section to review and settle your hospital bill. PhilHealth and HMO claims are processed here.

Payment & Clearance

Settle outstanding balances at the Cashier. Obtain your clearance slip before leaving the Nursing Station.

Final Release

Return hospital property (ID band, linens). Our staff will assist you to the exit. Take care and see you in good health!
